I-Cards are issued to full-time and part-time students and their spouse, as well as faculty, staff and full-time employee dependents. To obtain an I-Card, visit the Accounting Services office (KIM 130) and have your I-Number ready.
Lost or Stolen I-Card
If your I-Card is lost or stolen, report it immediately to the I-Card Office or Security Office. Upon notification, the I-Card will be disabled. There is a $10 fee to replace a damaged, stolen or lost I-Card ($20 for a Prox card). The University is not responsible for the inappropriate use of lost or stolen I-Cards, including any loss of I-Card Dollars or Dining Dollars, prior to being notified.
If you need to replace a card because of a name change, simply bring the old card to the I-Card Office, and it will be replaced free of charge.
Online Student I-Card Application
Online Students may obtain an I-Card by following the necessary steps (full requirements listed on application document). Not following these instructions may delay processing of I-Card Requests.
- Complete the 'Personal Information' section of the I-Card Application Form. Do not sign the application at this point.
- Submit a colored passport photo that is 2" X 2" in size.
- Include a letter from your ecclesiastical leader or sign and date the application in the presence of a Notary Public.
- Mail in the application.
