Student/Spouse/Parent Tax Return

Students may upload a hand-signed copy of the individual’s tax return. The 1040, 1040A, and 1040EZ along with any requested schedules may be obtained from the tax preparer or online software used to files taxes.

  1. Form 1040: *Hand signed multiple pages including schedules

    1. If there is any income on line 12 we also need Schedule C

    2. If there is any income on line 17 we also need Schedule E (must have Schedule K-1 as well)

    3. If there is any income on line 18 we also need Schedule F

  2. Form 1040A: *Hand signed, two pages

  3. Form 1040EZ: *Hand signed, one page

*The copy of the tax return must be hand signed by the tax filer or contain the necessary tax preparer information (tax preparer’s Social Security Number, Employer Identification Number, or Preparer Tax Identification Number; and has been signed, stamped, typed, or printed with the name and address of the preparer of the return).

Or, the individual may request a FREE Tax Return Transcript from the IRS via phone (1-800-908-9946), by appointment in an IRS office, online, or by mail.

When requesting a Tax Return Transcript, tax filers need to provide the following information:

  1. Social Security Number (SSN)

  2. Date of Birth

  3. Street Address, and

  4. Zip Code

Student/Spouse/Parent Verification of IRS Tax Non-filing

Verification of IRS Tax Non-filing may be requested from the IRS via phone (1-800-908-9946), by appointment in an IRS office, online, or by mail. Students who are unable to obtain the Verification of IRS Tax Non-filing may upload a hand-signed letter from the individual whom the form is being requested from certifying all of the following:

  1. The individual attempted to obtain the Verification of Non-filing from the IRS and was unable to obtain the required documentation.

  2. The individual has not filed an “input tax year here” income tax return and is not required to file.

  3. List the sources of any “input tax year here” income earned by the individual from work and the amount of income from each source. Attach a copy of the IRS Form W-2, or an equivalent document, for each source of “input tax year here” employment income received by the individual.
