Privacy Release Form (PRV)
Please be aware that due to Federal privacy laws, our counselors can only speak to YOU about your financial aid. You are able to authorize our office to give information about your financial aid to someone other than yourself, such as a parent or spouse. From your financial aid student portal, click on the "authorizations" tab, scroll to the bottom of the page and select the person you want to authorize from the drop down menu. If they are not listed, or if there is not a drop down menu, you will need to download and complete a "privacy release form" and bring it into our office.
GRE Fee Reduction Request Form
The Graduate Record Examinations (GRE) is a graduate-level admissions test. Whether you are planning to go to graduate or business school, or just exploring your options, taking the GRE can be an important step towards your future.
In order to request a fee reduction, you will need to complete the GRE fee reduction request form and submit it via mail or in person to the financial aid office. Once submitted, a financial aid counselor will prepare your GRE fee reduction certificate based on information from your current FAFSA. This certificate must be mailed with your GRE test application.
For students living within 30 minutes of campus, your certificate may be picked up 48 hours after submitting your request to our office. For online students or those living further than 30 minutes from campus, you are able to receive your certificate by mail, if specified on your form. Please make sure to include your current address on the form. More information about the GRE, including who should take the exam, can be found here.
Praxis Fee Waiver Request Form
Education majors will need to take the Praxis® test for teacher certification and licensing. If you qualify for a fee waiver, the test fees for up to three Core tests or one Praxis II® Subject Assessment may be waived. There are a limited number of fee waivers available, and only one waiver can be used during a testing year. All fee waiver requests are processed on a first-come, first served basis.
To apply for a Praxis® fee waiver, you must submit a completed Praxis® Fee Waiver Request form into the financial aid office. Waivers may be picked up 24 hours after your request is received by the financial aid office. Waivers must be submitted to the address on the bottom of the form.
To be eligible for a fee waiver, you must:
- Be currently receiving federal financial aid.
- Be enrolled in an undergraduate or graduate program.
- Meet income guidelines per the fee waiver request form.
- Be required to take the Praxis® test by an authorized score recipient.
